How do I add or modify a Thumbnail Shape for the Visual Index format, so that all thumbs get a fixed with with a dynamic height?
Also how do I add CSS styling (i.e. a line underneath) to these thumbs?

That's a format dependent thing and there isn't a format yet that does that - I'm really close to releasing one but I'm still on vacation until next Tuesday.
