Hi, today while working on the site i've got this strange problem.
on the browser tab instead of the website name it get display this:

i was working in my theme folder on index.php and style.css
what have i messed up?

That means it's not parsing - you would also not have any content. Can you show us the site?

Check your template because it's outputting the template TWICE. I have no idea what's going on there...
The good news is that it is working on the second pass which is why you see content. View your html code to see what I'm talking about...
the strange thing is that even if i switch to default theme i get the same error
This is mystifying. Did you save the template in your root index.php as well by any chance?

It's fixed. You had saved the template to /ndxzsite/plugin/index.php - which is not what should be happening.
