anyone have any clue on how to try to fix this? nothing was done to the site, woke up one morning a few days ago to this error on all the index sites on my server.

Yeah, it's been hacked. Do a search on the forum ('hack') for info about how to clean things up. You can save things...probably only takes 15 mins. And then, back up your site so should it happen again you can quickly deal with it.
